Title

Nonsurgical retreatment of teeth with apical periodontitis persisting after periapical endodontic surgery

According to most studies available in endodontic literature, the success rate of the apicoectomy is estimated at up to 80%. According to the same studies, the surgical retreatment with a complete healing after the failure of the first apicoectomy shows a significantly lower success rate, of about 36-59%. Tooth extraction is usually preferred in such cases. No studies regarding the success rate of orthograde retreatment of failed periapical surgery are yet published. The introduction of the dental operating microscope in the last years, has enabled the successful orthograde retreatment after periapical surgery failure, as well as the surgical endodontics. The use of MTA appears to enable a satisfactory apical seal of the wide remaining canal lumen after root-end resection. On the basis of a few case reports of orthograde retreatment following a failed apicoectomy, it will be demonstrated the technique and potential of endodontics retreatment. This treatment technique could be more extensively evaluated only after further histological investigation of the tissues around the neoapex, as well as after the completion of clinical long term studies.
